Boy Scout Troop 803

Medical forms are needed to attend many boyscout events.

This is the new form for scouts and adults that will be 100% effective next year and replaces the current 1, 2,&3 forms. It is a bit more thorough, but not difficult, and complies with the new requirement that medical forms must be renewed annually. We will get into more of a discussion about this when we start our new season. Everyone who needs a physical should use the new form
